according clive barker , doug bradley, earliest incarnation of pinhead appeared in hunters in snow, original 1973 play doug bradley in role of dutchman, undead inquisitor , torturer. later film titled forbidden, shot in 16 millimetre , in black , white, included prop in form of wooden block 6 nails in it, gave distorted shadow formations under different lighting angles. years later, during scripting of hellraiser, same design applied pinhead s face give same effect.

after being disappointed way material had been treated producers in underworld (which included scene in needles burst out of character s skull), barker wrote hellbound heart first step in directing film himself.
